GP Pathfinder Clinics – one of England’s largest and most innovative primary care providers – has been named winner of the “Best Solution to Manage Chronic Disease” award at the 2026 Health Tech Digital Awards, recognising its innovative digital programme designed to transform diabetes management in primary care.
The award celebrates GP Pathfinder Clinics’ digitally enabled population health approach, which uses primary care data, pharmacist-led medicines optimisation, and standardised digital workflows to proactively identify and support patients living with diabetes who have unmet care needs.
Through advanced EMIS data searches, patients are identified and prioritised based on clinical risk factors, enabling earlier intervention and more targeted support. The programme combines structured digital templates, automated follow-up pathways and evidence-based protocols to deliver consistent, guideline-aligned care at scale.
The initiative has already demonstrated measurable improvements in glycaemic control, cardiovascular risk management and preventative screening uptake, while helping practices manage increasing diabetes prevalence across diverse patient populations.
The scalable model highlights how effective use of digital tools and population health data can improve long-term outcomes for people living with chronic disease, while supporting sustainable delivery within primary care.
