- Think Healthcare: Denny Cross Medical Centre, a busy GP training practice in NHS Forth Valley, uses Think Healthcare’s cloud telephony platform to support clinical supervision, improve patient access, and protect frontline staff. By embedding call recording into its training programme, the practice now gives every GP trainee structured, evidence-based feedback from real patient interactions. Callback has improved access for patients, and reliable recordings have transformed complaint resolution. - Zanda Practice Management Software: Zanda is a cloud-based practice management platform purpose-built for allied health providers. Supporting over 40,000 users globally, Zanda unifies scheduling, clinical documentation including built-in AI, billing, payments, telehealth and reporting into a single secure system. By replacing fragmented tools with an integrated solution, Zanda reduces administrative burden, improves financial visibility, and enhances patient experience. From solo practitioners to multi-site clinics, healthcare providers use Zanda to streamline operations, strengthen compliance, and enable sustainable growth allowing clinicians to focus more time on care and less on paperwork.
- Best mHealth Application:
- Vivoo – AI-Powered Preventive Health Intelligence Platform: Vivoo is an AI-powered mHealth application that helps people understand their health before symptoms appear. By combining at-home wellness testing with intelligent mobile software, Vivoo translates biological signals into clear, actionable insights users can apply in daily life. Designed for accessibility and scale, Vivoo empowers preventive health engagement outside of clinical settings, helping hundreds of thousands of users make more informed health decisions through a simple, mobile-first experience.
- Best use of Artificial Intelligence:
- UCLPartners, AI for Urgent and Emergency Care: AI for Urgent and Emergency Care is an NHS England-supported collaboration between UCLPartners, Health Navigator and NHS North East London Integrated Care Board. The initiative uses AI to identify people at higher risk of unplanned emergency care and offers them personalised, phone-based clinical coaching to prevent crises before they occur. The initiative has been rolled out across North East London with more than 4,500 patients onboarded so far.
- Best Communication Solution:Â
- Think Healthcare (Focus Group): Think Healthcare’s cloud telephony platform has transformed patient communication at Denny Cross Medical Centre, a busy GP training practice in NHS Forth Valley. Reliable call recording, smart callback, and live coaching tools have improved patient access, resolved complaints fairly, and supported the development of GP trainees through structured, evidence-based supervision. Staff feel better protected, and patients get through more easily. With AI-powered sentiment analysis and call scoring now in development, Think Healthcare is taking communication quality in primary care from reactive management to intelligent, targeted improvement, at scale, across the NHS.
- Digital Transformation Project of the Year:
- West Hertfordshire Teaching Hospitals NHS Trust and Alertive: West Hertfordshire Teaching Hospitals NHS Trust’s Alertive project has transformed deteriorating patient care through EPR-integrated digital alerts that automate NEWS2 escalations. Replacing outdated bleeps with secure real-time messaging, it has enabled faster responses, better multidisciplinary collaboration, and standardised processes. Co-designed with clinicians and patients, Alertive is also improving patient safety, strengthening staff confidence, reducing cognitive load, and shifting care from individual responsibility to system-led.
- Best Elderly Care Technology Solution:
- Sage: Sage is an AI-powered care platform designed specifically for elderly care, enabling safer, faster, and more proactive support for older adults. By combining wireless nurse call, real-time insights, and intelligent monitoring, Sage improves response times, reduces fall risk, and enhances overall quality of life. The platform streamlines caregiver workflows while providing operators with actionable data to optimize care delivery. Supporting the full continuum from independent living to skilled nursing, Sage helps providers meet the complex needs of aging populations—delivering better outcomes for residents, empowering caregivers, and creating more resilient senior care communities.
- Best Healthcare Management Solution:
- Oushk Pharmacy, Oushk: Implementing a GPhC-aligned digital GLP-1 care pathway: Oushk developed a GPhC-aligned digital care model that embeds clinical governance directly into the delivery of online weight-management treatment. By integrating structured assessments, mandatory live video verification, pharmacist-led prescribing and enforced follow-up into its platform, Oushk operationalised regulatory standards at scale ahead of the December 2025 deadline. In 2025, the model supported more than 90,000 patients, demonstrating that digital healthcare can expand rapidly while maintaining consistent oversight, verified patient data and accountable clinical decision-making.
- Best Corporate Wellbeing Solution:
- Personify Health: Personify Health is a personalised wellbeing platform helping employers, insurers and distributors improve population health through prevention, behaviour change and better access to support. Working with global organisations, the platform brings fragmented wellbeing offerings into one simple, joined-up experience, driving strong engagement and meaningful health outcomes. Results include increased physical activity, weight loss among high-risk individuals, and clinically significant improvements in key health indicators such as BMI and blood pressure across large populations.
- Best Diagnostics Solution:
- TestDynamics: TestDynamics created a unique imaging platform, Satori, designed to make it easy for clinicians to consume AI insights – it serves as a bridge between third-party AI algorithms and healthcare providers. The company created the first holistic healthcare platform for the medical imaging and diagnostic sectors that merges multiple AI sources, systems, and perspectives into one solution so that radiologists can make decisions in real-time from unified intelligence. The platform is designed to enhance patient care outcomes, ensuring more accurate diagnoses and reducing clinician errors.
- Best Solution to manage Chronic Disease:
- GP Pathfinder Clinics Solution to Manage Diabetes: GP Pathfinder Clinics has implemented a digitally enabled population health programme to improve diabetes management and reduce long-term complications. Using EMIS data searches, patients with unmet care needs are proactively identified and prioritised based on risk, with support from pharmacist-led medicines optimisation clinics. Standardised digital templates and protocols ensure consistent, guideline-aligned care and enable automated follow-up. The programme has improved glycaemic control, cardiovascular risk management and preventative screening, while supporting a growing diabetes population. This scalable model shows how effective use of primary care data and digital workflows can enhance chronic disease management and improve patient outcomes across diverse populations.
- Best Remote Monitoring Solution:
- SafePatch by OGOOUE CORPS TECHNOLOGIES: SafePatch is a body-powered remote monitoring patch that provides continuous health and location monitoring without batteries or charging. Powered by body heat, SafePatch detects falls, immobility, and abnormal temperature, sending automatic alerts to caregivers and healthcare teams. Designed for elderly care, home monitoring, and remote healthcare, SafePatch enables early intervention, improves safety, and supports preventive care through autonomous, eco-designed technology.
- Best Solution for Community Care:Â
- Totalmobile’s Care at Home and Reablement Solution: Fife Council has transformed its Care at Home service using Totalmobile’s CareLink solution, a powerful digital platform for scheduling, mobile care delivery, and real-time workforce management. Since implementation, the service has increased weekly care hours by 1,713, delivered over 3 million visits and achieved millions in annual savings. CareLink has enabled safer staffing, improved continuity of care, and enhanced the patient experience. With live scheduling, mobile access, and ongoing innovation, including a new service user portal, CareLink is helping Fife deliver care of the future: smarter, more responsive, and more personalised community care at scale.
- Best use of Technology in Social Care:Â
- More Days Studio: More Days Studio is an evidence-led lifestyle and wellbeing platform for care homes. It generates a complete, CQC-mapped evidence trail as a natural output of the activities programme — individualised engagement records, outcome measurement via the Journey Score, and group-wide governance dashboards. Studio addresses the gap between the quality of engagement care homes deliver and their ability to prove it. Active in pilot across England, with partnership integrations with leading care management platforms, Studio is building the evidence infrastructure that the sector needs and regulators are demanding.
