Increase in usage and reported benefits as Thames Valley & Surrey Care Records merger progresses

Usage of the TVS Care Records and reported benefits are steadily increasing as the merger of four separate Shared Care Record systems progresses, creating a unified, regional platform across Buckinghamshire, Berkshire West, Frimley, Oxfordshire, and Surrey.

Between July 2024 and January 2025:

  • 30% more patient records were accessed as adoption rises among frontline staff.
  • Health and social care users increased by almost 7%, demonstrating wider engagement across all settings.

The integration is reducing duplication, improving triage, and streamlining clinical workflows. Reported benefits have included some GP surgeries seeing a 27% drop in calls from staff in other services requesting information, helping them manage demand more effectively.

Previously, different areas operated separate systems – including Connected Care (Frimley and Berkshire West), My Care Record (Buckinghamshire), Surrey Care Record, and Oxfordshire Care Summary.

This consolidation is set to be fully completed by the end of March 2025It is being led by the Thames Valley and Surrey Care Records Partnership, hosted by Frimley Health Foundation Trust.

TVS: A Data-Driven Care Record

The TVS Care Record, powered by Graphnet Health, provides real-time access to essential health data for health and care professionals, including patient histories, hospital admissions, and treatment records, breaking down barriers between organisations and improving patient outcomes.

The TVS platform also integrates the Johns Hopkins ACG® System and uses its PNG segmentation tool, which helps identify and stratify patient populations based on clinical risk. This allows clinicians to proactively triage and manage high-risk individuals, anticipate potential health crises, and allocate healthcare resources more effectively. By leveraging predictive analytics, the ACG System enhances care coordination, helps improve patient outcomes, and supports data-driven decision-making across the NHS.

The CIPHA population health platform integrates data from diverse sources—including hospitals, GPs, social care, mental health, community health, ambulance services, and more—to provide a comprehensive view of population health trends. This enables healthcare providers to identify at-risk individuals, monitor chronic conditions, and intervene early to prevent illness. More than 17 million citizens are now covered by CIPHAincluding those within the TVS Care Record, demonstrating the impact and scalability of integrated digital health systems.
